Health Waiver
LIABILITY WAIVER, RELEASE & ASSUMPTION OF RISK

PLEASE READ CAREFULLY BEFORE SIGNING
This Agreement applies to any person entering, visiting, participating in activities at, receiving services at, or otherwise using the premises.
The premises may be used for dance classes, rehearsals, performances, physical training, workshops, healing sessions, wellness activities, retreats, meditation, breathwork, bodywork, and other artistic, physical, educational, healing, or wellness-related activities.
1. ACKNOWLEDGMENT OF RISK
I understand that entering the premises and participating in or observing activities may involve risks, including slips, falls, collisions, strains, sprains, physical injury, overexertion, dizziness, aggravation of pre-existing conditions, emotional discomfort, and other foreseeable or unforeseeable injuries or losses.
I voluntarily enter and use the premises and knowingly assume the risks associated with my presence and participation.
I understand that participation is voluntary and that I am responsible for deciding whether an activity is appropriate for me. I agree to stop participating if I experience pain, dizziness, distress, or other symptoms that make continued participation unsafe.
2. NOT A MEDICAL CLINIC
I acknowledge that the premises are not a hospital, medical clinic, psychiatric facility, emergency facility, or other healthcare facility.
Healing, wellness, bodywork, meditation, breathwork, movement, or other services provided at the premises are not intended to replace professional medical or mental-health diagnosis or treatment.
Unless a provider separately identifies themselves as a licensed healthcare professional acting within the scope of their license, services offered at the premises should not be considered medical care.
I am responsible for seeking appropriate medical or mental-health advice when needed.
3. RELEASE OF LIABILITY
To the fullest extent permitted by law, I release and discharge:
SHAYA VÉ NETA LLC; Ido Tadmor, founder and artistic director; Steven Yablok, owner of the Theatre; Complexions Contemporary Ballet Academy; the owner and operator of the premises; and their respective owners, members, managers, directors, employees, instructors, teachers, practitioners, contractors, agents, volunteers, representatives, affiliates, successors, and assigns
(collectively, the “Released Parties”)
from claims, demands, damages, losses, costs, or liabilities arising from or relating to my presence at, use of, or participation in activities at the premises, including claims arising from the ordinary negligence of a Released Party, to the fullest extent such claims may legally be released.
Nothing in this Agreement waives liability that cannot legally be waived.
4. PERSONAL PROPERTY AND THEFT
I am responsible for my personal belongings while on the premises.
To the fullest extent permitted by law, the Released Parties are not responsible for lost, stolen, misplaced, or damaged property, including clothing, jewelry, cash, bags, electronic devices, equipment, or other personal belongings.
5. TEACHERS, PRACTITIONERS AND SERVICE PROVIDERS
I understand that teachers, instructors, healing practitioners, wellness practitioners, facilitators, contractors, and other service providers may independently provide services at the premises.
Unless expressly stated otherwise in writing, their presence or use of the premises does not mean that SHAYA VÉ NETA LLC or any other Released Party guarantees, endorses, supervises, or assumes responsibility for their qualifications, methods, advice, services, or conduct.
Each teacher, practitioner, contractor, or service provider is responsible for their own services, conduct, qualifications, licenses, certifications, insurance, equipment, representations, and compliance with applicable laws and professional standards.
6. SAFETY AND EMERGENCIES
I agree to follow reasonable facility rules and safety instructions.
I understand that the premises are not an emergency medical facility. In the event of a serious injury or emergency, emergency services may be contacted.
I am responsible, to the extent permitted by law, for medical or emergency care provided to me.
7. MINORS
If the Participant is under 18 years of age, a parent or legal guardian must sign where required by law.
Any waiver or release relating to a minor applies only to the extent permitted by applicable law.
8. GOVERNING LAW
If the premises are located in California, this Agreement shall be governed by California law.
If any provision is found invalid or unenforceable, the remaining provisions shall remain in effect to the fullest extent permitted by law.
ACKNOWLEDGMENT
I HAVE READ AND UNDERSTAND THIS AGREEMENT.
I understand that I am voluntarily assuming risks and releasing certain legal claims.
I acknowledge that the premises are not a medical clinic or healthcare facility and that I am responsible for my own health, safety, participation, and personal property.
By signing below, I voluntarily agree to the terms of this Liability Waiver, Release & Assumption of Risk.
